BankProv takes our obligation to protect your data very seriously. In order to process your data request, we need some basic information from you. The information requested in this form is essential to BankProv’s efforts to verify you are who you claim to be, to locate your data within our systems, and to determine whether we may have a lawful reason to deny your request.

We will work to process all verified requests within 45 days of its receipt. If we require more time, we will inform you of the reason and extension period in writing. When your data is ready, we’ll email you instructions to securely download a copy of it.

If you are a California or EU resident, using the form below you can request a copy of your data, download your data when it’s ready, or request to delete some of your data.

This may require additional follow-up to ensure confidence that this request can be fulfilled. We will work to fulfill this request in a timely manner that complies with the legal requirements in your jurisdiction.
